Invariable when we do the demolition for a bathroom remodel project, we find hidden damage that can not be seen until we do the demo. The most common is wood rot from moisture (bathrooms being the wettest room in your home). The second most common is wood damage from termites.
I can not think of a single bathroom we’ve torn apart without finding some level of hidden damage. The older the home the more damage we generally find.
The only effective strategy I know for not being surprised by hidden damage is to expect to find it. I recommend homeowners allocate 10% of their total bathroom remodeling budget to repairing hidden damage. If yours if very light and it costs you less, then your surprise is a good one.

In some ways this is a more difficult issue as home owners often do not understand the ramifications of what they desire and contractors sometimes don’t realize homeowners don’t know.
Here is a concrete example. We’re in discussion with a homeowner right now who decided he wants the doorway into his bathroom to be wider. The extra cost of this surprised him as he failed to realize this requires reframing the door. The contractor working with him has such a natural and intuitive understanding of this need to reframing that he failed to specifically mention it, because how could anyone not know?
This resulted in surprises on both sides.
The effective strategy for you as a homeowner is when requesting a change (even if it’s to relocate the bathroom exhaust fan) is to ask how this change affects the overall plan and the cost.
